In order to connect to a MySQL database you need a JDBC MySQL Driver.
Download the driver and unpack it somewhere, then click the "Load" button of the Database Reader's dialog and select the "mysql-connector-java-xxx.jar". In the drop down box you can select - as far as I know - every mysql driver. Then everything should work as expected.
