Have you just recently patched the machine?
The most common cause of this problem is that you've patched a machine
whilst Disksuite had a disk in maintance mode. As devices in maintenance
mode will not get written to, this means that one side of your / mirror
will get patched, whilst the other won't. When you come to reboot if
you boot off the un-patched side then things will break badly in much
the same way as you've seen.
The fix is simply to boot off the other side of the mirror. eg, in the
above you're booting off /pci at 1f,4000/scsi at 3/disk at 0,0 which looks like
disk0 on an E220/420, so presuming your mirror disk is the other internal
just try using "boot disk1" to fix the problem.
Once you've done that, fix whatever is broken in disksuite, and then
implement some form of monitoring so that you detect failed disks in
disksuite sooner!
